Master Consultation Form
The Master Consultation Form page allows administrators to manage all consultation forms used within the system. These forms can be used to collect important information from customers before or during appointments.

Page Overview
The consultation forms are displayed in a table format, where each row represents a form. From this page, administrators can search, review, and manage consultation forms efficiently.
If no forms have been created, the table will display a message indicating that no data is available.
Search
A search bar is available at the top of the page.
Use this field to quickly find a consultation form by entering keywords such as:
- Form name
- Form type
The table updates automatically based on the search input.
Consultation Forms Table
The main section of the page displays the list of consultation forms with the following columns:
Name
This column displays the name of the consultation form.
The name helps identify the purpose of the form, such as intake forms, medical history forms, or service-specific questionnaires.
Form Type
This column indicates the type of consultation form.
Form types may define how and where the form is used within the system, such as during booking, before appointments, or at check-in.
Status
This column shows whether the form is currently active.
Active :
The form is available for use and can be assigned or filled by customers.
Inactive :
The form is not available for use but remains saved in the system.
Actions
The Actions column provides options to manage consultation forms.
Common actions may include:
Edit : Modify the form structure, fields, or settings.
Delete : Remove the form from the system.
These actions allow administrators to keep forms updated and relevant.
